
Eligibility
To be eligible for Texas's Case Management for Children and Pregnant Women (CPW) services, you must be:
Residency
You must be a Texas Resident
Medicaid Enrollement
Must be eligible for Texas Medicaid
Need for Service
You must require assistance to access necessary medical, social, educational, and other services.
Voluntary Participation
You must want to receive case management services
Patient Status

Children: Birth through 20 years of age with a health condition or at risk for one that limits their ability to function compared to peers.

Pregnant Women: Any age, with a medical or psychosocial condition that puts them and their fetus at a higher risk for pregnancy or postpartum complications.
